Player A bets $4000
all other players fold except player B raises all in
Player A calls the all in
In the mean time the dealer takes all the discarded cards and mixes them back into the deck thinking no one called,how would you handle this

Sounds like one of those rare situations that occurs in a home game. It's a missdeal so the loser is the holder of the best cards. It's the dealers fault so there's nothing you can do about it. Chips would be returned to the position before the hand and the next hand would be dealt.
